• By now, you should have gotten our intake forms sent to you via email. Please make sure that this form is completed. If you have not gotten this form via email or  are having trouble completing it, please email us at info@blueheronpsych.org

​

• There may be additional forms which will be sent to your email address prior to your appointment. Please complete at your earliest convenience.

​

• We ask that you update your account prior to the visit to include a valid credit card, per our office policies under "Payment" section. 

​

• Please email a copy of your driver's license to our encrypted, HIPAA complaint email address at info@blueheronpsych.org

​

• If you have previous medical records, we welcome you to either send to us via the HIPAA compliant email address at info@blueheronpsych.org, fax to (845) 584-5901 or bring them to your appointment if you are coming to the office. 

​

• For patients seeing us virtually: the Zoom link was sent to you with the initial appointment confirmation. If you are having difficulty retrieving the link, please reach out to us at your earliest convenience in order to avoid last minute technical issues. 

​

• For patients coming to our Rhinebeck office:

  • Please note that our office is located on the second floor and there is a flight of stairs one needs to climb. If you have difficulty climbing a flight of stairs, please contact us ahead of time to make arrangements. In some cases, we may recommend converting the visit to a virtual one. 

  • We ask that you arrive 5 minutes prior to your appointment time. Please do not arrive more than 5 minutes early to your appointment to ensure privacy of both you and the previous patient. 

  • Please bring all current medication bottles with you to your appointment. 

  • Please see the map below for a detailed location of our office, as well as a photo of our building with a green arrow noting where the entrance is, facing h. We are located at Astor Square, above the Karate Academy.  ​
